If you are here, you’re having trouble with this abstract horror of Lovecraftian proportions. The Flesh Lips, or Lustful Lips, is the final boss encounter at the end of the Brookhaven Hospital area. The normal Hospital, by the way. Not the… otherworld.
After Laura dukes you into the door like the cheeky bugger she is, the Flesh Lips boss will stalk you in the cutscene, preparing you for the first major boss fight in Silent Hill 2 Remake. He or she is a chunk of flesh with arms and legs trapped in a rectangular metallic cage.
Conserve Ammo
Before fighting Flesh Lips, you must conserve ammo before the boss fight. Make sure you scavenge enough healing items and shotgun shells throughout the hospital before committing. To learn where the shotgun is, check out the guide here.
This boss requires a ton of ammo as you need it to destroy its metallic defenses.

Just so you know, the Remake bosses have a major twist to their encounters. It isn’t like the original, where James can just go in the corner and shoot until it dies. Flesh Lips is the first taste (eugh) of Silent Hill 2 Remake’s reworked boss fights.
The boss has two phases. Yep, you read that right. The original version does not fall to the ground. In the Remake, he will drop down once you have depleted enough of its health. Its moveset will change and will surprise new and old players. This forces you to change tactics once it happens.
Observe Its Moveset
The first phase requires you to dodge and shoot when he hangs on the ceiling. Check the ceiling tiles; dust will fall, and sound FX will warn you of its next attack.
Its first phase is easily telegraphed, but Flesh Lips’ swinging move can cover a wide distance. So be careful.
Shoot its Hands and Exposed Flesh!
Once you get a good feel for its move set, you can now plan your attack. This fight isn’t like the first Pyramid Head boss, where you just wait for the alarm bells to ring. You have to kill it.
When it hangs on the ceiling, you can shoot its exposed hands, staggering it. A handgun is recommended as it has a better aim.
Once it falls to the ground, it won’t move for a few seconds. This is the perfect time to hit those shotgun shells to expose its flesh. Repeat until you get to its second phase.
The Last Stand
You know you’re doing great when he drops down and breaks the other side of the room. This will give you a more expansive space to move around and evade its new moves. There are also healing items and ammo found around the corners.
This is its last stand. He will unbind himself from its metallic cage and launch a series of spiked attacks. Flesh Lips will use metal rods from its once-protective cage to spin and spike you from great distances. Make sure to keep your distance.
It will let out a gurgling scream for its final act of desperation. This will render you immovable, so wait for it to stop and back away quickly. After that, keep adding pressure by shooting its exposed flesh with your remaining handgun and shotgun bullets.
A cutscene will play out when you finally beat him. However, that doesn’t mean the nightmare is over. But congrats, either way.
