To solve the combination lock, follow the clues found in Room M2.
There's a cryptic note with several lines leading to a number.
The note reads:
“1 year they've been watching me
So many pills they fed me
When a loud hour comes
I tremble every time the light falls”
- 1 - The first clue, "1 year they’ve been watching me," directly gives you the number 1.
- 6 - In the sink, you’ll find six pills, matching the line "So many pills they fed me."
- 2 - This number comes from adjusting the clock in the room. If you fiddle with it, the clock will stop at 2, meaning it's a “loud hour.”
- 2 - The light flickers twice, matching the line "I tremble every time the light falls."
In short, the combination lock code is 1-6-2-2.
After solving the combination lock, your next task is to locate the Lapis Eye Key, which unlocks another part of the chained box. Start by heading to Exam Room 4 on the third floor.
Inside, you’ll find a mannequin sitting in a wheelchair. Use the Instrument of Force (a hammer) to break the mannequin’s arm. The item is in the same room. Doing so will reveal a spot where the key can later be placed.
To retrieve the key, go to the first floor and go to the Medical Records Room. Inside this room, you will find the Key of Bliss hidden in a hole in the wall.
Once you have the Key of Bliss, return to the mannequin in Exam Room 4. Use the key to extract the Lapis Eye Key from its eye socket.
The final part of the puzzle requires solving a keypad code. First, head to the Director’s Office, but you’ll need to find the key to unlock it before entering.
This key can be found in the Observation Room, which you’ll access by pushing a movable laundry cart to reach a vent. Crawl through the vent and fight off any enemies you encounter.
In some situations, it may be best to simply avoid combat and run past enemies, especially in tight corridors where fighting might become risky.
Once you clear the area, grab the Director’s Key from in front of the television set. Return to the Director’s Office and find a ladder to the third floor, where a machine is waiting.
This machine will only work once powered by a generator. Follow the wire from the machine until you reach the generator room, turn it on, and then go back to the machine. When activated, the machine burns the keypad code 9-6-5-9 onto a stretcher.
Now that you have the combination lock code (1-6-2-2), the Lapis Eye Key, and the keypad code (9-6-5-9), return to the Chained Box and use all three to unlock it. This will reveal the contents inside and allow you to progress.
